Managing an ongoing condition often involves several conversations over time. A follow-up visit gives you a place to review changes, ask about your care plan and discuss what has been difficult between appointments. Your provider determines the timing of visits, examinations, testing and any treatment changes based on your circumstances.

The practice offers chronic condition management alongside preventive visits, sick visits and in-house lab draws. If you also see a specialist, bring their contact information and relevant records you have so you can discuss how the pieces of your care fit together. Ask the office how to arrange any transfer of records.

When scheduling, say whether you are establishing care, following up on results or discussing an existing plan. If you prefer to continue with the same provider, consult the usual provider schedules and confirm availability with your chosen office.

Choose a location and confirm the visit

Our Sunrise office is at 8395 West Oakland Park Blvd, Suite E & F. Our Coconut Creek office is at 5300 West Hillsboro Blvd, Suite 100. Both provide primary care and in-house blood draws; provider schedules and lab start times differ.

Self-pay options are available. The published rate list shows an $88 follow-up visit, effective January 1, 2024 and subject to change. Confirm current pricing and any separately billed tests. If you have insurance, ask the office to confirm your specific plan.

Can I discuss several ongoing conditions at one visit?

Tell the office what you need to discuss when scheduling, and bring your most important questions. Your provider can help prioritize the visit and determine whether additional appointments are needed.

Can follow-up happen by telehealth?

Anling Health offers telehealth for appropriate concerns. Call the office to ask whether the particular visit is suitable for video or needs an in-person examination or testing.

Will every follow-up require blood work?

The need and timing of testing depend on your individual care plan. Your provider determines which tests are appropriate. Ask about costs and preparation before a planned draw.
