Choose the office that fits your day

Both offices offer primary care, in-house blood draws and EKGs. Providers work different days at each location. If you prefer a particular provider or language, mention it when arranging your visit and check the team’s usual schedules.

Published office hours are Monday–Friday, 8:30 AM–5:30 PM, with a lunch closure from noon to 1:30 PM. Saturday visits are by appointment. Check holiday hours before traveling.

Prepare for the conversation

A little preparation helps your provider understand what matters to you. Before your appointment, gather:

  • A list of the concerns you want to discuss, with your most important question first.
  • Your medication and supplement names, doses and any known allergies.
  • Relevant recent test results, specialist information and previous primary care records you already have.
  • Your insurance information, if applicable, and any school or work forms you need completed.

Ask the office how to provide records and what identification or paperwork to bring. Tell the team if there is a deadline on a form. Do not send medical records or detailed health information through the callback form; the office can explain the appropriate way to share them.

If blood work may be part of your visit, ask whether preparation is needed before arriving. Fasting is not required for every test. Follow the instructions from your provider or the office.

Confirm the cost before the visit

You can request a visit without insurance. Our published self-pay list shows a $100 new patient visit and an $88 follow-up, with separate rates for after-hours visits and EKGs. These published rates are effective January 1, 2024 and may change; confirm the current rate when scheduling.

Laboratory work is billed separately. Ask about any additional tests and charges before they are performed. If you have insurance, the office can help confirm participation in your specific plan; coverage and out-of-pocket cost depend on that plan and the services involved.

Is a callback request a confirmed appointment?

No. A callback asks the office to contact you. The team will discuss your request and confirm the date, time, location and provider. Call the office if you need to discuss availability directly.

Can I request care in another language?

The providers and office team speak English, Mandarin, Cantonese, Sichuanese and Spanish. Availability varies by provider and location. Tell the office your preferred language when arranging the visit. Our 中文服務頁面 also provides practice information in Chinese.

Which type of visit should I ask about?
